Their role is to identify vulnerabilities, help individuals access appropriate support and provide information that can help inform criminal justice decisions.
The service screens for up to 13 different vulnerabilities and supports people both within police custody settings and as they prepare to leave prison, helping them access services such as housing, healthcare and wider community support.
The team wanted to move beyond a referral-led model, where individuals were only assessed when identified by police officers.
Many vulnerabilities can be hidden, and the service wanted to offer support more consistently by screening everyone entering custody rather than relying on referrals alone.
This change dramatically increased demand, requiring practitioners to engage with four to five times more people while maintaining a consistent standard of service across different locations and practitioners. At the same time, the organisation needed stronger reporting processes and more effective ways to provide information back to NHS England.
